Many serious accidents have occurred because a machine was started when it should have been completely turned off to allow for maintenance or repair. These accidents are usually the result of energy that was not released properly. Lock-out-tag-out (LOTO), which ensures that machines are shut off until an authorized person releases them, helps to prevent these accidents.
The LOTO system consists of a variety of locks tags, locks, and other devices that are used to isolate, secure or isolate the machine from an electrical source. All "affected", "authorized", and "qualified" personnel should be taught about the use of lockout/tagout devices. These devices include, but aren't only restricted to chains, locks and padlocks. Each device is uniquely identified and labeled, containing the date and time of its application and the name of the authorized individual and the reason for the lockout or tag-out (repair or maintenance, for example). It must be stated clearly that these devices are not to be removed until the LOTO procedure is complete.
